CEAT Engagement Report Form
Use this form to record any group conversations/ presentations you have done. Be sure to collect comment cards and the email address of anyone who wants to receive updates. Completed forms should be returned to Julie Miller, jlb51@scasd.org. If you would like an administrator or board member to assist you with your presentation, contact Julie.
